Help shape the EMR to openEHR synchronization toolkit (open consultation)

I have published a practical toolkit for synchronising data between an EMR and openEHR: a step-by-step guide, ready-to-use templates (an intake and decision form and a data contract), and a fully worked example using blood pressure. It is built to be picked up and used, and shared as a living set that gets better as more people put it to work.

So consider this an open consultation. Does the approach and the order of the steps work? Is anything missing? Does it hold up in your own setting, including other EHRs or openEHR platforms? Short comments are as welcome as detailed ones.
